  • Enter Serial No. & PIN of your card
  • Click LOGIN to fill your Application Form
  • Note that only submitted forms will be processed.

  • Attach a recent passport photograph (not more than 6 months old)
  • Maximum passport size should be 20kb
  • Passport must be of Jpeg format
  • Passport must be taken with red background

    • Double Entry is not allowed. It would lead to automatic disqualification of such an applicant.
